The Invisalign Promise and Reality
Invisalign promises a faster path to straight teeth than traditional braces, and for many patients, it delivers. But “how long?” is the first question we hear at Bonin Dental Care, and the honest answer is: it depends. Treatment duration varies widely based on your starting bite, the degree of crowding or spacing, how consistently you wear your aligners, and how your teeth respond to pressure.
Most patients complete Invisalign in 6 to 18 months. Some achieve results in as little as 3 months (simple cases), while complex cases may take 24 months or longer. Understanding the factors that influence your personal timeline helps you set realistic expectations.
Typical Case Durations by Complexity
Simple cases (minimal crowding, slight spacing, no bite issues) often resolve in 6 to 9 months. Think a patient with mostly straight teeth who wants to close small gaps or fix minor rotations. These cases progress steadily with few adjustments.
Moderate cases (mild to moderate crowding, some spacing, minor bite discrepancies) typically take 12 to 15 months. Most patients fall into this category. Teeth move gradually in response to aligner pressure, and the jaw needs time to adapt.
Complex cases (severe crowding, significant spacing, major bite misalignment, or cases where teeth need to move both vertically and horizontally) can take 18 to 24 months or longer. These cases might also require extraction, bone grafting, or initial fixed braces to establish a foundation before aligners take over.

What Extends Treatment Time?
Several factors can push your timeline beyond the initial estimate:
Inconsistent wear is the most common culprit. Invisalign aligners must be worn 20 to 22 hours daily for teeth to move as planned. If you’re removing them frequently or forgetting to wear them, your treatment stalls. Each month of reduced wear can add weeks or months to total treatment time.
Slow bone response varies naturally between patients. Some people’s bones remodel quickly in response to orthodontic pressure, while others move more slowly. Age, bone density, and genetics all play a role. There’s not much you can do about this, but Dr. Bonin can adjust your treatment plan if movement is slower than expected.
Aligner compliance and comfort matter too. If an aligner doesn’t fit perfectly due to missed wear or improper insertion, the next stage won’t progress smoothly. Poor-fitting aligners must be re-made or adjusted before moving forward.
Complex interarch relationships (how your upper and lower teeth bite together) sometimes require refinement phases or additional aligners beyond the initial set. If your bite isn’t quite right after 18 months, you might need 6 more months of refinement.
Extractions or prior dental work can complicate timelines. If you need a tooth removed before starting Invisalign, that adds time. Similarly, if you have large fillings or crowns, space management becomes more intricate.
The Refinement Process
After you complete your initial aligner series, Dr. Bonin will assess your bite and tooth position. If the result is excellent, you’re done. But most cases benefit from a “refinement” phase.
During refinement, new aligners are fabricated to address remaining minor discrepancies. You might need 10 to 20 additional refinement aligners, which could take another 2 to 6 months. This is a normal part of the process and doesn’t mean something went wrong; it’s a fine-tuning step that delivers a more refined final result.

The Retention Phase
Once your teeth are straight, the work isn’t quite finished. Teeth naturally want to drift back toward their original position, so retention is critical.
Most patients wear a fixed retainer (a thin wire bonded to the back of your upper and lower front teeth) combined with removable retainers (custom trays worn nightly, similar to Invisalign aligners). Some patients wear removable retainers indefinitely; others transition to wearing them several nights per week after the first year.
The retention phase isn’t part of your “Invisalign timeline,” but it’s essential to maintaining your results. Patients who skip retainers often see their teeth drift back within months.

Perfect aligner wear is paramount. Wear your aligners 20 to 22 hours daily. Remove them only to eat, drink, and clean your teeth. Consistency compounds over time.
Prompt response to treatment adjustments matters. If Dr. Bonin recommends an extraction or additional refinement, moving forward quickly prevents delays.
Healthy gums and bone support faster tooth movement. Maintaining excellent oral hygiene throughout treatment (brushing twice daily, flossing, regular cleanings) protects your periodontal health and supports efficient tooth movement.

Real-World Timeline Example
Here’s what a typical moderate case looks like from start to finish:
Month 1-2: Consultation, treatment planning, custom aligner fabrication, initial wear begins.
Month 3-6: First aligner series (usually 10 to 15 aligners, one every week or two weeks depending on the case).
Month 6-15: Remaining aligners in the initial series. Periodic check-ins with Dr. Bonin every 4 to 8 weeks to monitor progress.
Month 15-18: Completion of initial aligner series. Assessment by Dr. Bonin. If minor adjustments are needed, refinement phase begins.
Month 18-21: Refinement aligners (10 to 20 additional trays). Fine-tuning of bite and tooth position.
Month 21 onward: Transition to retention phase. Fixed retainer placed; removable retainers prescribed and worn nightly.
Total active treatment: roughly 18 to 21 months. Retention is ongoing but not considered “treatment” anymore.
Invisalign vs Braces Timeline
Traditional braces typically take 18 to 24 months, similar to complex Invisalign cases. The key differences are that braces are more aggressive (they can apply stronger forces) and require no patient compliance regarding wear. However, many patients prefer Invisalign because it’s less visible, easier to clean, and more comfortable, even if the timeline is comparable.
Setting Realistic Expectations
When considering Invisalign, focus on the realistic range for your case type, not on the best-case scenario you’ve heard. Moderate cases often take 12 to 18 months, not 6. Complex cases take 18 to 24 months. Planning for these timelines and being pleasantly surprised if treatment finishes early beats the reverse.
Also remember that having straight teeth is a permanent advantage. Six more months of aligners is a small price for a lifetime of improved smile, bite, and oral health.
